Life After Fifty has been awarded a grant of $56,600 on behalf of GSC Six4Six Community Granting Initiative to support their Social Prescription Pilot project. The purpose of the project is for Life After Fifty to become the referral destination in Windsor, through Social Prescription, for primary care practitioners prescribing physical and social activity to socially isolated senior patients to improve their mental health. Funding will be for a Social Prescription Coordinator to work with local Community Health Centres and their teams of Primary Care Practitioners to launch the pilot. The intent through this one-year pilot project is to demonstrate the effectiveness of this program and secure long term funding through other sources.
